I get severe esophageal spasms. I am on Nitro patches for it. Yes, Nitro. They do stop the spasms and they most certainly mimic heart attacks. Before my Dr put me on them, he too was concerned about my heart, but like you, all tests were normal. I have reflux as well. Mind if I tell you what happens with spasms? When you secrete too much stomach acid, it will splash again the esophagus and cause these spasms. Sometimes, the Nitro doesn't work and I get a little scared. I have endoscopes every 6 months. They are scary and it sounds like you have those spasms. Taking an ulcer medication slows down the rate of secretion of stomach acid. Good luck.
